Scoring Form Directions
Read the response, looking for health-enhancing content, concepts and skills. Then, score the response, first, on Health Content and Concepts, then, on Health Literacy Skills. Score the response from 1 to 4 on the CONTENT dimension, based on the scoring rubric for Health Content and Concepts. Information on appropriate Content and Concepts is provided by the Form's Help links.
If the paper is not scorable, indicate that decision by marking:
- BK: Response is Blank
- URR: Un-Readable Response (i.e.: blurred, too light, etc.)
- NSR: Non-Scorable Response (i.e.: off-topic, illegible, foreign language, etc.)
Next, if one or more SKILLS dimensions are listed, score those 1 to 4, according to the scoring rubric, or NONE if there is no evidence of the skill. More information on Health Literacy Skills is provided by the Form's Help links.
When all scores are entered, submit your scores for this response by choosing that SUBMIT button which is appropriate.
